The Illinois/Midwest Orthopedic Network (ION/MON) Data System will be rightfully and lawfully accessible only to persons authorized by ION/MON to access.

Information in Authorized User Sections are solely meant to be used to support and assist Users in their work efforts concerning ION/MON patients.

You acknowledge that the Authorized User Sections contains Confidential Information. You agree to protect the confidentiality of the Confidential Information with at least the same degree of care employed with respect to your own personal confidential or proprietary information and not to disclose any such Confidential Information to anyone other than Persons who have a legal, authorized or legitimate reason to have such Confidential Information.

You are solely responsible for (1) maintaining the strict confidentiality of the user identification, passwords and codes ("Authorized User Information") assigned to You or created by You; (2) charges, damages or losses that may be incurred or suffered as a result of Your failure to maintain the strict confidentiality of Authorized User Information; and (3) promptly informing ION/MON in writing of any need to deactivate or replace your Authorized User Information due to security concerns.

Please be aware, to protect the security of our System, if it is detected that no login activity has occurred within 180 days, we will disable that account. You may request for your account to again become enabled, and we will send you information you will need to update your credentials. We appreciate your efforts in helping us to secure our System.

You agree to notify ION/MON immediately as soon as You become aware of any potential or unauthorized use of Your Authorized User Information.
